By submitting this form and signing below, the customer confirms that the information provided—including the declared value and item description—is accurate to the best of their knowledge. The customer understands and agrees that the item is being submitted to HUBERT Inc. strictly for repair or service. HUBERT Inc. is not an insurer of the item. For items manufactured by HUBERT Inc., we will insure the item while it is in our possession. In the unlikely event of loss, theft, or damage caused by our negligence, we will repair or replace the item up to the value declared on this form. If no value is listed, it will be assumed to be less than $100.00.
For Items Not Manufactured by HUBERT Inc.: We take great care in every repair and treat all items with the highest level of attention and professionalism. However, for items not originally made by HUBERT Inc., we cannot accept responsibility for damage that may occur during service—particularly to gemstones, diamonds, or structural elements. Because we did not manufacture these items and may be unfamiliar with their internal construction, such work is done entirely at the customer’s risk. If desired, we can refer customers to a third-party insurer who may offer coverage for such repairs.
Return shipping will be handled by HUBERT Inc. and insured to the declared (or assumed) value. The customer is responsible for shipping costs, even if the estimate is declined. HUBERT Inc. is not responsible for any item left unclaimed for more than 90 days after notification.
